Treatment of choice for atypical mycobacterial lymphadenitis is complete excision of the affected node, without need for antimicrobial chemotherapy, depending on the site and if rupture has not occurred. If surgery is not an option, in cases of cutaneous disease or if the condition is chronic, combination treatment with a macrolide and/or quinolone-containing regimen can be used, with varying degrees of success. Antimicrobial resistance is often a problem with the atypical mycobacteria group.
