Adolescent athletes with back pain often have the acquired traumatic form, where a fracture occurs through a normal pars interarticularis, usually at L5 (41). Sports that involve repeated lumbar hyperextension, such as gymnastics, dancing, figure skating, diving, soccer, football, hockey, and lacrosse, are particularly associated with traumatic spondylolysis and spondylolisthesis (3,36). In one study, 40 of pediatric soccer players with spondylolysis remembered maximum velocity kicking as a triggering event for their back pain (42). The high dysplastic developmental type of spondylolisthesis also often becomes symptomatic during adolescence and usually involves L5-S1 (41). This type of spondylolisthesis is more likely to be associated with bladder and bowel problems and L5 nerve radiculopathy (43).

Osteitis Pubis

Inflammation of the pubic symphysis and the surrounding muscles may result from repetitive stress, or imbalance of the adductor muscles, iliotibial bands, or hip flexor muscles. Athletes who perform running, kicking, or rapid lateral movement, such as sprinters, soccer players, and hockey players, are more susceptible to this injury. A number of different conditions may cause pain in the middle of the foot. Again, acute injuries are obvious and easily recognized with appropriate X-rays. The most common serious cause of chronic foot pain in active children is a stress fracture of the metatarsals. Stress fractures are overuse injuries found in many different sports, ranging from football and soccer to dance and gymnastics, and can occur in other parts of the body as well. They are typically treated with casting and rest. Although the initial X-ray may not show the fracture, follow-up X-rays should demonstrate healing bone to confirm the diagnosis.

These sports-specific injuries are seen in multiple sports, including cutting activities (football, soccer), repetitive rotational activities (golfers, martial artists), dancers, and skaters.

Fat Camps

At all weight-loss camps, diet is combined with about three to four hours of exercise per day (Ellin 2005). More and more camps are moving toward a recreational sports model of exercise as opposed to the regimented intentional exercise of earlier programs (Holt et al. 2004 222-3). For example, physical activity at Wellspring's general and adventure camps is provided through fun group sports, such as soccer and hiking. This newer model of physical activity reflects general physical-education findings that children are more likely to stick to an exercise routine when they enjoy it and represents a departure from earlier boot camp models for weight reduction.

Groin Strain

Muscles involved in this problem include the pectineus, adductor brevis, adductor longus, adductor magnus, and gracilis. Groin strain often occurs in sports that require pivoting and quick changes of direction, such as soccer and hockey. The injury may range from mild pain in the adductor muscles to severe pain, tenderness, and inflammation in this area. Damage is usually localized to the musculo-tendinous junction, about 5 cm from the pubis.

Apophyseal Injuries

Avulsion fractures results from indirect trauma caused by sudden, violent, or unbalanced muscle contraction, and are most commonly associated with sports such as soccer, rugby, ice hockey, gymnastics, and sprinting, that involve kicking, rapid acceleration and deceleration, and jumping. Whereas in adults this mechanism of injury typically causes a muscle or tendon strain, in skeletally immature athletes the consequences are more serious, because of the inherent biomechanical weakness and subsequent separation of the apophyseal region. Intensive training exposes the epiphyseal plate to repetitive tensile stress while simultaneously enhancing muscle contractility and power. The inherent weakness at the epiphyseal plate, combined with the increased functional demands placed on the musculature, may predispose athletes to subsequent avulsion injury. Once the injury has occurred, the degree of bony displacement is restricted by the periosteum and surrounding fascia.

Definition of Terms

Aerobic exercise does require oxygen for energy. This is observed during exercise that is less intense but of longer duration. This energy system is primarily used during events lasting longer than several minutes, such as a two-mile run or the Tour de France bicycle race. The potential does exist that one can use both systems, as in soccer, where a match requires ninety minutes of continual activity with short intense bursts of effort.
