Long protocol

The long protocol is also called the down-regulation protocol and — as you probably guess — this protocol takes a while to get through. The reason for this is that you actually start this protocol in the second half of the cycle before your planned treatment cycle.

You begin the protocol by taking a GnRH agonist every day, as either an injection or a nasal spray. This drug shuts down your body's own production of follicle-stimulating and luteinising hormones (hence the term down-regulation). Such action helps promote the growth of multiple follicles after you start the FSH injections and also stops you from ovulating before the planned egg collection. After a couple of weeks you start FSH injections and these continue until the largest follicles measure at least 18 mm in diameter, after which you have a single injection of hCG to trigger maturation of the eggs ready for retrieval.
