Scapulothoracic musculature.5 Properly developed core strength, providing sufficient core stability, has been suggested as a necessity for maintaining correct posture and alignment of lumbar and pelvic regions during all movement and particularly in sport because it enables powerful and coordinated extremity movements. Inadequate core strength results in poor core stability and may decrease bio-mechanical efficiency, as well as increasing the risk of injury.


Treatment consists of daily physical therapy focusing on core body strengthening and hamstring flexibility. Weight control measures are important to discuss with overweight adolescents. Heating before activity may help to increase stretch of muscles. Pilates and yoga are excellent activities for this group. Pain medications may be used but are not particularly helpful for relieving discomfort. Proper posture, workspace computer ergonomics, use of a lumbar support pillow for sitting and shoewear with good arch supports during prolonged walking may benefit adolescents with muscular low back pain.

Parents must put their baby to sleep on his back to help prevent SIDS, despite the chance that he may develop a flattened head. Operator Posture And Stance

Factors relevant to operator posture are weight in relation to gravity the contact with the floor or table the position of his head and all other aspects of weight distribution consistent with the ability to transmit forces in the best way for each technique. The term stance relates more to the position of the operator in relation to the table, and the proximity to the patient. Although the two are to some extent interdependent, there are differences. Posture is the state of balance and coordination of musculoskeletal and visceral systems. Factors of safety for patient and operator, and effectiveness in technique by the use of efficient posture. Many operators have suffered injury from strains in treating patients. Much of this could have been avoided by more care over posture. Good posture allows the operator to reduce strain on his own structure, and to reduce the possibility of excessive force being applied unwittingly to the patient.

Specifically, to plan an arm trajectory toward an object, the CNS first must locate the position of the object with respect to the body and represent the initial position of the arm. Recordings from single neurons in the parietal cortex and superior colliculus in awake monkeys have significantly contributed to our understanding of how space is represented. There is some evidence that in the parietal cortical areas there are retinotopic neurons whose activity is tuned by signals derived from somatosensory sources. Their visual receptive field is modified by signals representing both eye and head position. This result suggests that parietal area 7a contains a representation of space in body-centered space. Neurons representing object location in body-independent (allocentric) coordinates have also been found in the parietal cortex and in the hippocampus (Andersen et al. 1993).

Postural problems

Posture is the term used to describe the alignment of the body. Good posture means that the body is balanced and the muscle work required to maintain it is minimal. Poor posture means that the body is out of balance and certain muscles must contract strongly to maintain the


Postural instability is created by a pattern of weakness, muscular tightness, and standing alignment changes that diminish the patient's ability to control their center of gravity during transfers and gait. A common presentation is that of a stooped forward posture of the upper body with tight anterior chest wall musculature and a crouched lower body posture. A series of stretching exercises designed to diminish kyphosis of the thoracic spine and increase flexibility in the pectoralis major and minor muscles can lead to improved upper body posture and upper limb function. In the lower aspect of the body, strengthening of the lumbar paraspinal musculature and stretching of the hamstring and hip flexor muscles can be used to improve posture. It is important not only to stretch the key muscles in patients with poor posture, but to also strengthen the appropriate muscles to achieve good biomechanical alignment. Spatial Perception

First, neglect occurs in all sensory modalities (Bisiach and Vallar 1988). The multimodal nature of neglect indicates that what has been damaged is not simply a set of sensory maps but a high-level, supramodal representation of space. Second, neglect occurs with respect to a variety of spatial reference frames. A patient with right parietal damage is typically unaware of objects on the left side of space but left may be defined with respect to the body, or with respect to the line of sight, or with respect to the object to which the patient is attending. Moreover, these spatial impairments are dynamic, changing from moment to moment in accord with changes in body posture (Mosco-vitch and Behrmann 1994) and task demands (Behrmann and Tipper 1994).
