As it is for any patient, the therapeutic relationship can be very important and special for older patients. Older adults tend to have few opportunities to talk about their problems; whereas the common societal perception ofolder adults is that they talk freely about their problems (particularly medical ones), many of our patients indicate that they tend not to complain, because they are very aware of how continued complaints can turn people away. Some older adults feel embarrassed about needing mental health care, and do not tell friends and family they are feeling depressed. Thus, the therapist is usually the one person to whom they can talk freely about their worries. In addition, older patients have learned many new ways to cope with their moods and problems by the end oftreatment, and they tend to feel anxious about their ability to use these new skills on their own. Cognitive therapists who work with older adults must make extra efforts to ensure that older patients feel able to manage these new skills on their own. This requires preparation early on in treatment to ensure that patients understand that they will be using these new skills on their own.

In addition, as soon as older patients grasp the new strategies or skills, therapists should have them begin work on the thought record and activity scheduling forms. Some patients at the Over-60 Clinic program are given the opportunity to attend monthly booster sessions to reinforce the new skills when they no longer feel depressed.
