It has been speculated that the presence of an estrogen-like 1,2 diarylethane moiety in azoles may relate to their antifungal activity, and synthesis of azole derivatives emphasizing this moiety produced compounds with antifungal activity (Massa et al. 1992).

Thymus serpyllum was found to be highly active against various species of Penicillium, Fusarium and Aspergillus (Agarwal and Mathela, 1979 Agarwal et al., 1979). It could be demonstrated that thyme oil (T. vulgaris) inhibits both mycelial growth and aflatoxin synthesis of Aspergillus parasiticus (Tantaoui-Elaraki and Beraoud, 1994).

Other head and neck manifestations include parotid gland enlargement, orophar-yngeal candidiasis, nasopharyngeal neoplasms (Kaposi or lymphoma), oral ulcerations, and oral hairy leukoplakia. Nontender parotid gland enlargement may be a heralding sign of HIV infection (14) and may be uni- or bilateral. Enlargement is most often attributable to the benign lymphoepithelial cyst, which is characterized by uni- or multilocular cyst masses, similar to the benign lymphoepithelial lesion found in Sj gren's syndrome. In contrast to the Sj gren's lesion, the cysts noted in HIV infection are thought to originate from ductal pressure induced by intraparotid lymph node hyperplasia associated with generalized lymphadenopathy (15). Side Effects of Corticosteroids

The side effects of corticosteroids are numerous and very common. They include Cushing's syndrome, fluid retention, increased appetite and weight gain, truncal obesity (skinny arms and legs but increased fat on back and stomach), moon face (fat cheeks), stretch marks, acne, growth retardation, bone-weakening calcium loss, avascular necrosis, muscle weakness, poor blood sugar control (diabetes), cataracts, increased intraocular pressure, increased infections, oral and vaginal thrush, atherosclerosis, extra hair growth, and mood changes. In addition to these common side effects, corticosteroids also may cause high blood pressure, inflammation of the pancreas, and pseudotumor cerebrae (increased pressure in the brain, associated with severe headaches and visual problems).

Differential Diagnosis

In addition to the clinical symptoms of anemia, iron deficiency produces a microcytic, hypochromic anemia. Serum iron is low. Total iron-binding capacity is elevated. In severe xerostomia, the tongue may exhibit atrophy of the lingual papillae and erythema that is frequently secondary to atrophic candidiasis. Central papillary atrophy is a localized form of depapillation secondary to candidiasis (Fig. 46).

Clinical Manifestations

NME is the hallmark finding in glucagonoma syndrome. It is characterized by a polymorphous eruption that most commonly presents as scaly, erythematous papules and plaques with superficial erosions. The lesions typically are in a perioral distribution on the face (Fig. 9), but also involve the perineum, lower abdomen, thighs, buttocks, and less commonly the distal extremities. NME has been reported to be the presenting complaint in approximately two-thirds of patients with this tumor, but there are only rare cases of glucagonoma without NME ever occurring. Stomatitis, glossitis, dystrophic nails, and alopecia can also be seen. The eruption is frequently misdiagnosed as seborrheic dermatitis or intertrigo and can also resemble pemphigus foliaceus, acrodermatitis enteropathica, chronic mucocutaneous candidiasis, or psoriasis.

Allergic rhinosinusitis

A more effective approach to nasal congestion and inflammation is the use of a topical nasal corticosteroid spray. This class of agent is the most potent and effective modality for the treatment of allergic rhinitis and is effective against all of the manifestations of this disorder (8). Common side effects include local nasal irritation and occasional thrush. Less common, but possibly important in the case of long-term use, are an increase in incidence of glaucoma, cataracts, and osteoporosis. These drugs diminish the inflammatory influx of cells into the nasal mucosa and diminish the strength of the local allergic response.

Safey Issues Related to Inhaled and Systemic Corticosteroids

The NHLBI document states that inhaled corticosteroids are the most effective therapy for long-term control of mild, moderate, and severe persistent asthma and are well tolerated at recommended dosages. The overwhelming evidence demonstrating their efficacy far outweighs the small risks of adverse effects. Local adverse effects of ICS include oral candidiasis, dysphonia, reflex cough, or bronchospasm with inhalation. Spacer devices are recommended to prevent dysphonia and oral candidiasis. The key recommendations for reducing the potential adverse effects of ICSs are (i) administer ICS drugs with holding chambers or spacers (ii) patients should be instructed to rinse their mouths with tap water after each dose (iii) use the lowest effective doses (iv) consider adding a LABA to a low or medium dose of ICS rather than increase ICS dose (v) monitor growth in children and (vi) recommend supplemental calcium (1000-1500 mg day) and vitamin D in postmenopausal women receiving ICS therapy (2).

Complications To Heart And Blood Vessels

Endocarditis, an infection of the tissues in the heart, usually a heart valve, is a progressive disease characterized by frequent embolization (obstruction of blood vessels) and severe heart-valve destruction that can be fatal if not treated. This disease can result from repeated injection of the infective agents into the blood system, usually from nonsterile needles and or unusual methods of injection. Infective endocarditis is highly prevalent among drug abusers and should be suspected in any needle-using abuser who shows such symptoms as the following fever of unknown origin heart murmur pneumonia embolic phenomena blood cultures that are positive for Candida, Staphylococcus aureus or enterococcus, or Gram-negative organisms.

Clinical Manifestationssystemic

An extensive discussion of the systemic manifestations of HIV AIDS would be quite lengthy and beyond the scope of this chapter. Note will be made of the PHI, which presents with fever, myalgias, pharyngitis, arthralgias, headache, and malaise. Candida albicans is a common organism found in the oral cavity flora that causes candidiasis in certain clinical situations. Candida has a capsule and forms true hyphae and pseudohyphae. It adheres to mucosal surfaces and is capable of superficial mucosal invasion. Factors that contribute to oral candida proliferation include uncontrolled diabetes mellitus, antibiotic therapy, and any condition that causes immunosuppression. Clinical features of candidiasis include white, cheesy plaques that can be wiped off with a tongue blade. The tongue and buccal mucosa are frequently infected, but tonsillar and pharyngeal lesions are not uncommon and may extend to the esophagus and larynx. Similar to C. albicans, S. cerevisiae has been demonstrated to have an EBP (Burshell et al. 1984 Feldman et al. 1982), as does P. brasiliensis as discussed earlier. The studies in S. cerevisiae showed it to have a 60-70 kDa protein that specifically bound E2 with a Kd 1 nM with a binding capacity of 2,000-4,000 fmol mg of protein. Interestingly, S. cerevisiae was thought to possess an endogenous estrogen ligand, but carefully performed studies showed the presence of estrone in the components of culture media (e.g., molasses and Bacto-Peptone) that could be enzymatically converted to E2 by the organism (Feldman 1988). Future Directions

As is evident from the information presented in this chapter, the field of fungal endocrinology encompasses a number of different areas and shows that these simple eukaryotic organisms can indeed have true hormone receptor interactions. Additional studies are needed in the search for endogenous hormones that regulate mating of higher fungi, such as the dermatophytes, or Coccidioides. The influence and interactions of mammalian hormones on pathogenic fungi remains an area for study that has thus far given indications that host hormones can positively or negatively influence the pathogenesis of a fungal infection. For example, hormonal influences on C. albicans have been implicated in the pathogenesis of candidal vaginitis (Tarry et al. 2005). The question of whether these interactions occur through a fungal receptor-mediated process has begun to be addressed for C. albicans, where genomic expression studies have demonstrated upregulation of genes in the organism related to drug...

Skin problems Intertrigo, seborrheic eczema, and thrush are common in the thick heavy skinfolds of severely obese children. Pink or pale cutaneous striae, distinct from the purplish striae resulting from thinning of subcutaneous tissues in Cushing's syndrome, are common on the abdomen and upper limbs and may be a source of embarrassment. Hirsutes (abnormal facial and body hair) occurs particularly in adolescent girls with polycystic ovarian syndrome, which is associated with obesity and insulin resistance. Acanthosis nigricans, a velvety, pigmented, thickening of the skin usually at the back of the neck, is another important marker for insulin resistance, affecting up to 90 of children with type 2 diabetes mellitus.

What is esophagitis

Esophagitis is inflammation of the lining of the esophagus that is diagnosed by a barium X-ray or endoscopy. Generally, it is not a diagnosis that can be made by a doctor in the office through a history or physical examination. The inflammation is usually caused by reflux of acid that burns, damaging the esophagus. However, medications can sometimes cause esophagitis, for example, aspirin or over-the-counter pain relievers, iron pills, potassium pills, some antibiotics, and certain drugs used to treat osteoporosis such as alendronate sodium. Rarely, infections of the lining of the esophagus such as yeast (Candida) or certain viruses can cause esophagitis. But again, generally most esophagitis is caused by acid reflux disease.


There are a number of alternative and unproven treatments that people claim are effective in treating ADHD. These include restricted diets, allergy treatments, megavitamins, biofeedback, anti-motion sickness medication, antioxidants, chiropractic adjustment and bone realignment, treatment for yeast infection, and so forth. There are also a variety of natural products advertised on the Internet and other places that claim to cure the symptoms of ADHD (CHADD, July 2001).


Initially, echinacoside, a caffeic-acid glycoside that showed weak antibacterial activity, was thought to be the active ingredient. Further work showed this was not the case.33 In a series of elegant experiments spanning more than a decade, M. L. Lohmann-Matthes, H. Wagner, and colleagues have steadfastly expanded our knowledge of how Echinacea works.34-38 Early on, this group pinpointed polysaccharides from the aqueous extracts of E. purpurea as the active fraction. Further work showed that the effective polysaccharides were cell wall-derived arabinogalactan and two fucogalactoxyloglucans. They developed a plant-cell-culture system whose supernatant provided them with a solution of the polysaccharides that could be standardized. Then they applied this purified extract in a host of carefully executed experiments. Mutation of the transcription factor Foxp3 which is the pivot molecule in the generation of (peripheral) regulatory lymphocytes induces a specific severe multifocal disease, the IPEX syndrome, which includes immune dysregulation with severe autoimmune enteropathy, poly-endocrinopathy, and X-linked inheritance.

Candida albicans The yeast that causes the infection called candidiasis (THRUSH), often found within the vagina or on other mucous membranes (such as the inside of the mouth). The infection is also known as moniliasis. Because it is so commonly found in the body, it is a problem only when it grows too abundant due to changes in the mucous membranes. Almas and co-workers compared the antibacterial effects of Miswak extract with eight commercial mouth rinses. They evaluated the antimicrobial effects on Pyogenes Faecalis, Mutans Streptococci, Candida Albicans plus Aureus and Epidermidis staphylococci by determining the inhibition zones. In their study, none of the solutions was considered a gold standard they compared the antimicrobial effect of Miswak with that of each mouth rinse and the antimicrobial effects of the eight mouth rinses with each other. According to their results, mouth rinses containing chlorhexidine had the greatest antibacterial effects, while mouth rinses containing cetylpyridinium had moderate effect Miswak extract had a low effect ( Almas et al 2005).


Dermatitis is a diseased condition that appears as inflammation of the skin. The gum tissue is much like the skin on your face and shares similar histologic (cellular structure) characteristics. Diseases affecting your skin are lichen planus, candidiasis, psoriasis, pemphigus vulgaris, and pemphigoid. Candidiasis is produced by candida, a fungal yeast infection. It is also referred to as thrush or acute pseudomembranous candidiasis. The roof of your mouth and cheeks are covered with white patches that when wiped off, leave a very sensitive, reddened, ulcerative area. This condition can cause more plaque retention and poor oral hygiene conditions, thus creating a severe gum problem.

Skin Conditions

Chelitis is the presence of fissures and red, scaly skin at the corners of the mouth and lips. This is usually due to moisture collecting at the corners of the mouth, but can also be complicated by infection from bacteria or the yeast Candida. The application of a mild steroid cream is useful, along with treating infection when present.

More recently, a 2004 review of five cohort studies further confirmed these findings and concluded that there is now sufficient evidence to show that diet does indeed influence longevity and that the optimal diet for the prevention of both coronary heart disease and cancer is likely to extensively overlap with the traditional Mediterranean diet (Trichopoulou & Critselis 2004). Normal Microflora

The microflora of the colon dramatically increases to a concentration of 1011-1012CFUgm-1. This bacterial load accounts for up to 50 of the volume of colonic content. Although the colonic microflora comprises more than 400 different species it is predominantly anaerobic including Bacteroides, Fusobacterium, Bifidobacterium, Lactobacillus, Enterobacter and coliforms, and other facultative anaerobes (Staphlococcus and Candida species).


Patients who require PN are often predisposed to infectious complications. The catheter hub is often the entry site, with skin flora such as Staphylococcus epidermidis, Staphylococcus aureus, or Candida being the most common organisms, along with gram-negative enteric bacteria possibly from bacterial translocation. Antibiotic treatment through the central line is often successful without replacement of the catheter using antibiotic combinations such as vancomycin and gentamicin or with an antibiotic lock.

Infection Viral

HIV is a retrovirus that infects CD4+ lymphocytes and monocytes. It is transmitted by sexual contact, blood transfusions, needle-stick injuries, maternal fetal transmission across the placenta, and maternal breast milk consumption. Acute prodromes of HIV infection include fever, malaise, cervical lymphadenopathy, and pharyngitis. Initial presentation is often confused with infectious mononucleosis and diagnosis is often delayed. These symptoms are usually self-limiting and resolve in approximately 7 to 10 days. Opportunistic infections with diseases such as HSV, CMV, and candida are the hallmark of this infection as CD4+ counts plummet.


Hydro-alcoholic extracts have been shown to have antibacterial, antiviral and antifungal activities. The in vitro antifungal activity of calendula flower extracts has been investigated against Aspergillus niger, Rhlzopus japonicum, Candida albicans, C. troplcallsand Rhodotorula glutlnis. Calendula extract showed a high degree of activity against all fungi and the inhibitory effect was comparable to that of standard antifungals (Kasiram et al 2000). A flower extract has been shown to inhibit trichomonas. The oxygenated terpenes are thought to be the main active compounds (Gracza & Szasz 1968, Samochowiec et al 1979). A 70 hydro-alcoholic extract demonstrated virucidal activity against influenza virus and suppressed the growth of HSV (Bogdanova et al 1970). Calendula flower extract has also been shown to possess anti-HIV activity in vitro (Kalvatchev et al 1997).

CMV disease in HIV is usually reactivation of a latent, previously acquired infection in patients with CD4 count of less than 100 cells mm3 (19) The typical primary CMV infection, presenting with a mononucleosis-like syndrome characterized by fever, pharyngitis, lymphadenopathy, and malaise, is not usually encountered in the setting of HIV infection. In the pre-HAART era, CMV reactivation disease could be asymptomatic or cause fever and constitutional symptoms or end organ disease, occurring in 40 of AIDS patients. The most common organ involved is the retina, in 85 of cases, with the GI tract accounting for another 10 . In the HAART era, incidence of new CMV-reactivation disease has decreased dramatically and is usually seen with CD4 counts less than 50 cells mm3. Vaginitis is an inflammation of the lining of the vagina. It is caused by a hormonal imbalance during postmenopause or postpartum by irritations from allergies or irritating substances and chemicals or by bacteria, candidiasis, or yeast infection, and trichomoniasis. The symptoms include itching, pain, and a white discharge. It is recommended sugar and possibly fruit juices be avoided in the diet as yeast thrives in that particular environment.
